What is Health Monitoring?

Health Monitoring at home involves qualified nurses providing skilled, evidence-based care according to an individually developed care plan developed in consultation with our clinical team.

Blood pressure and heart rate monitoring
Blood glucose monitoring (diabetes)
Oxygen saturation and respiratory rate
Weight monitoring and fluid status
Pain assessment and management
Cognitive and functional status tracking
Skin and pressure area assessment
Falls risk assessment
Documented clinical trends over time
GP communication when thresholds are reached

Who benefits from this service?

Health monitoring is particularly valuable for clients with heart failure, COPD, diabetes, chronic kidney disease, atrial fibrillation, or any condition where early detection of deterioration can prevent hospitalisation. Also valuable after medication changes or following hospital discharge.

Our RN conducts a thorough assessment and develops a detailed, individual clinical care plan.

We establish a visit schedule matched to clinical need and coordinate with your GP and specialist team.

Regular nurse visits deliver the clinical care, fully documented and communicated to your medical team.

Ongoing review and escalation

Clinical status is monitored at every visit. Concerns are escalated to your GP promptly, not at the next scheduled appointment.
