Wound care at home is appropriate for people with post-surgical wounds, chronic wounds (diabetic foot, leg ulcers, pressure injuries), anyone who has had a fall resulting in a wound, and people with conditions that impair healing such as diabetes or peripheral vascular disease.
How it works
Clinical assessment
Our RN conducts a thorough assessment and develops a detailed, individual clinical care plan.
Schedule and coordination
We establish a visit schedule matched to clinical need and coordinate with your GP and specialist team.
Skilled nursing care
Regular nurse visits deliver the clinical care, fully documented and communicated to your medical team.
Ongoing review and escalation
Clinical status is monitored at every visit. Concerns are escalated to your GP promptly, not at the next scheduled appointment.
🩺 Note: Wound infection can escalate quickly, particularly in clients with diabetes or circulation issues. Our nursing staff are trained to recognise signs of concern and escalate to the GP promptly, we do not manage complex wound complications independently.
